VERIFICATION IS MANDATORY THREE forms of residence verification documents. The most common are a driver’s license, HOA Bill, and a utility bill. Other common documents are mortgage/lease agreements, voter registration cards, or insurance documents. See below for full list of accepted documents. No more than one utility bill can be used for verification purposes. Documents must be dated after February 1 (does not include driver’s license). If you anticipate any issues in providing these documents, please contact our Player Agent. List of accepted forms of residence verification: Three documents must be submitted. Only 1 document per category may be used. Group One 1. Driver’s License 2. School records 3. Vehicle records (i.e., registration, lease, etc.) 4. Employment records 5. Insurance documents Group Two 1. Welfare/child care records 2. Federal records (Federal Tax, Social Security, etc.) 3. State records 4. Local (municipal) records 5. Support payment records 6. Homeowner or tenant records 7. Military records Group 3 1. Voter’s Registration 2. Utility bills (i.e., gas, electric, water/ sewer, phone, mobile phone, heating, waste disposal) 3. Financial records (i.e. loan, credit, investments, etc.) 4. Medical records 5. Internet, cable, or satellite records Each of the 3 forms must be highlighted with the following info: Parent's first & last name Address Billing Date or Date in which document is valid Company that issued the bill.
