§ THREE forms of residence verification documents. The most common are a driver’s license, HOA Bill, and a utility bill. Other common documents are mortgage/lease agreements, voter registration cards, or insurance documents. See below for full list of accepted documents. No more than one utility bill can be used for verification purposes. Documents must be dated after 2/1/2018 (does not include driver’s license). If you anticipate any issues in providing these documents, please contact our Player Agents. List of accepted forms of residence verification: Three documents must be submitted. Only 1 document per category may be used. Group One 1. Driver’s License 2. School records 3. Vehicle records (i.e., registration, lease, etc.) 4. Employment records 5. Insurance documents Group Two 1. Welfare/child care records 2. Federal records (Federal Tax, Social Security, etc.) 3. State records 4. Local (municipal) records 5. Support payment records 6. Homeowner or tenant records 7. Military records Group 3 1. Voter’s Registration 2. Utility bills (i.e., gas, electric, water/ sewer, phone, mobile phone, heating, waste disposal) 3. Financial records (i.e. loan, credit, investments, etc.) 4. Medical records 5. Internet, cable, or satellite records Each of the 3 forms must be highlighted with the following info: Parent's first & last name Address Billing Date or Date in which document is valid Company that issued the bill. In addition to highlighting- each form will need the "category" written in pen or on a sticky note: We will have highlighters at DICKS, but it is best to come prepared or your wait could be longer... IF this is your first season playing with LRLL: An original copy of a government-issued birth certificate. Photocopies are not allowed. Please note that if you did not play with LRLL in the spring 2018 season, you will need to provide a birth certificate again since your child did not play consecutive spring seasons.
